What is the Difference Between a Hypertrophic and an Atrophic Scar? Orlando, FL

As we go through life, it is common to experience scars. Scars can appear due to an injury, surgery or even acne. There are different kinds of scars; two of them are hypertrophic and atrophic scars. These scars can be aesthetically and functionally impairing, but their characteristics and treatments differ. Understanding the difference between them can help you choose the right treatment option.

Hypertrophic Scars

Hypertrophic scars are raised, thick and red. They develop when the body produces too much collagen during the healing process, which results in the overgrowth of scar tissue. These scars are typically confined to the boundaries of the original wound and do not spread beyond it. Hypertrophic scars can occur in any skin type but are more common in people with darker skin tones.

Some common causes of hypertrophic scars include burns, surgical incisions, trauma and piercings. They can also occur due to underlying medical conditions like acne or chickenpox.

Atrophic Scars

Atrophic scars are the opposite of hypertrophic scars. They are depressed, sunken and often have a pitted appearance. Atrophic scars happen when the body does not make enough collagen while healing, which causes the loss of tissue. Acne, chickenpox or injuries involving loss of skin tissue are the usual causes of these scars.

Several types of atrophic scars exist, including ice pick, boxcar and rolling scars. Ice pick scars are small, deep pits resembling an ice pick’s holes. Boxcar scars have a well-defined edge and are more expansive and shallower than ice pick scars. Rolling scars are wide and shallow, with a wave-like appearance.

Treatment Options

The treatment options for hypertrophic and atrophic scars differ depending on their characteristics. Hypertrophic scars can be treated with corticosteroid injections, silicone sheets, cryotherapy or laser therapy. These treatments can help to flatten the scar, reduce redness and itching and improve its appearance.

Atrophic scars, on the other hand, are more challenging to treat. Depending on the type of atrophic scar, treatments may include chemical peels, dermal fillers, microneedling, laser resurfacing or subcision. These treatments can help smooth out the texture of the scar, make more collagen and fill in the depression.
